TRANSITIONS™ is a home, beautifully decorated and specially designed for those who have difficulties with daily living due to memory loss. With only 14 individual apartments, our home is big enough to offer an active environment, but small enough so that no one is forgotten.

TRANSITIONS™ is a single-story residence. Our warmly decorated interior features its own kitchen and family-style dining room, a cozy fireplace and saltwater aquarium, living room and an inviting enclosed patio.

In addition, our Progressive Care program allows residents with early signs of memory loss to remain active and involved and then transition into a more intensive program when their care and need for more attention increases.

Our one-bedroom and studio apartments feel comfortable and familiar because our residents decorate their surroundings with their own treasured possessions.

With an extraordinary staff-to-resident ratio and a full activity calendar, our residents remain active and involved.
