Memory Care
Farmington has been a pioneer in providing care for those with Alzheimer’s disease and other related dementias in a residential setting. We recognize that the right environment could help diminish agitation and confusion. In fact, Farmington Centers was the first elder care company in the state of Oregon to design and build a facility specifically for people suffering from Alzheimer’s disease or other forms of memory loss.

We’ve also developed Transitions . . .
A flexible program that’s designed to meet the ever-evolving personal care needs of those displaying the initial signs of memory loss. As their needs become more advanced, so does the level of care and attention we provide.

The following services are included in the base rent:

• Caregiving staff on-site 24 hours a day
• Three nutritious homestyle meals daily
• Weekly housekeeping and personal laundry services
• 24-hour beverages and snacks
• Assistance scheduling medical appointments
• Social and emotional support
• All utilities (except personal telephone)
• Cable TV in common areas
• Coordination of transportation needs
• Full calendar of activities
• Routine apartment maintenance

Personal Care and Service Needs
We utilize a point system so that each individual is only charged for the services that are needed. We reassess after residents have been with us for 30 days, and then again every 90 days, or sooner if necessary to meet their needs.

Assistive services may include but are not limited to:

• Managing difficult non-combative behaviors
• Assistance with eating
• Assistance with hygiene and dressing
• Assistance with bathing and mobility
• Additional housekeeping services
